Dear Families and Staff,
In an effort to make our contact tracing efforts more efficient, we ask that you please report any positive COVID-19 cases using this Google Form moving forward rather than notifying your building principal or school nurse directly. The information submitted on this form will be kept confidential. After submitting the form, I, your building principal or the school nurse, will contact you for additional information.
During the winter and spring breaks, please continue to use this form to report positive cases. When you report a positive case during the break, you will be contacted within a couple of days should we need additional information. We also encourage you to reach out to individuals with whom the positive individual has been in close contact within 48 hours of the onset of symptoms or of testing if the individual is asymptomatic.
